Check Sheets 

Check Sheets are forms that allow us to quickly and easily record data in a visual manner. For maximum benefit, be sure to use operational definitions and test them before first use. Keep separate sheets for different users and different shifts. The example shown is used to track three kinds of manufacturing defects on a 24-breaker fuse box. It clearly shows where the problems are adding up, and establishes a strong focus on improvement. Check sheets provide a powerful method for investigating the physical nature of product defects. The form has three parts; 1) an indication of who collected the data and when, 2) a legend which identifies the different symbols used to indicate problems or faults, and 3) a pictorial display or diagram of the item.

Using the insights gained from this Check Sheet, operators and engineers can sit down together to redesign the item for improved manufacturability. They may also come to understand which of their machines need to be monitored with greater vigilance, and what kinds of equipment would be better suited to the task.
